Your Member ID Card

Every member of AmeriHealth Caritas Florida will get an AmeriHealth Caritas Florida member ID card. This card shows that you are a member of AmeriHealth Caritas Florida. Carry it with you at all times. You may need to show your AmeriHealth Caritas Florida ID card and Medicaid gold card:

To providers when you get health care.
If you go to the hospital.
At the pharmacy.

If you lose your AmeriHealth Caritas Florida member ID card, call AmeriHealth Caritas Florida Member Services at 1-855-355-9800. You will get a new card mailed to you right away.
